This service is designed to support DDD individuals in leading independent, meaningful lives by providing hands-on help, emotional encouragement, and skill-building in day-to-day activities. It's not just about meeting needs—it's about empowering each person to thrive on their own terms.

What Is Community-Based Support?

Community-Based Supports is a Medicaid-funded, individualized service for adults with developmental disabilities. It allows individuals to receive direct support from a trained staff member who assists them with their personal, social, and practical goals.

At ZINEniac, we take time to understand each person’s unique support plan and deliver services that match their abilities, strengths, and ambitions. Our support workers become trusted partners on their journey—whether that means helping with a morning routine or encouraging new hobbies.

Services May Include

✅ Assistance with daily living activities (dressing, grooming, hygiene)

✅ Support with meal preparation, shopping, and light household tasks

✅ Building communication and social interaction skills

✅Personal organization (time management, task planning)

✅ Companionship and emotional support

✅ Help accessing community resources and events

✅Support with recreation, hobbies, or creative interests

✅ Guidance with transportation or safe travel training

✅Support with using technology and devices

Where Services Are Provided

  1. In the individual’s residential home or supported living setting.

  2. At the ZINEniac studio, equipped for creative and social engagement.

  3. In community settings (e.g., library, coffee shop, local park).
